When you become aware of a member leaving the Bargaining Unit through resignation, retirement, promotion, etc., please have the member complete this form and forward the form by fax: 313-532-7306, mail: 24270 W 7 Mile Rd, Detroit, MI 48219 or email, to the Union as soon as possible.
